About the conference

Population ageing is one of the key challenges facing health and social care systems across Europe. What approaches have been proven successful in countries with similar historical experiences? What strategies can help ensure dignified ageing in the face of evolving demographic trends? And how can governments, professionals, and non-profit organisations collaborate effectively to provide quality and accessible care for older adults?
The conference will offer a summative overview of the current state of elderly care in the Czech Republic while drawing inspiration from the international context. It will highlight emerging trends in the care organisation, showcase best practices, and explore sustainable solutions for the future.
This year's event focuses on sharing experiences from countries with similar historical developments. Experts from different fields will present functional models of care, ways of more innovative approaches to cross-sector collaboration, and specific steps for the development of elderly care in our Central European region.

Registration of participants will take place between 8:30-9:00.

The conference fee is 1,150 CZK per person. A 50% discount is given to the third participant from the same organisation.
The fee includes conference admission, refreshments, and participation in one of the workshops.

Where Does Outreach Care End? Exploring Boundaries and Opportunities in End-of-Life Support

What limitations do outreach social services face when supporting clients with palliative needs or those receiving palliative care? And how can professionals navigate these challenges effectively?

This workshop is intended for providers of outreach social services who encounter end-of-life care in their practice. The aim is to reflect on the boundaries in delivering this care. We will focus on situations where the palliative needs of the client are perceived by the care team as not being optimally met. Participants will have the opportunity to discuss and share experiences, good practice in working with boundaries, the limits they are encountering, and reflect on the challenges and questions they currently face.

Participants will have the opportunity to explore selected case management tools, including force field analysis, the Eisenhower matrix, and risk analysis. This workshop is designed for professionals interested in incorporating case management—either in its entirety or through selected elements—into their work with clients. The aim is to expand your toolkit with techniques that offer a fresh perspective on complex client situations, potentially revealing new and alternative pathways toward resolution.

Community as Medicine: Why and How to Implement Social Prescribing

Although the Czech Republic offers a range of activities for older adults, these are often underutilised by those who would benefit most from them.

Social prescribing connects healthcare with psychosocial interventions by enabling doctors to “prescribe” specific activities that patients actually do. This workshop will introduce the concept and implementation of social prescribing in the Czech Republic and other European countries. The workshop will explore the key facilitators and barriers to this approach and collaborate on developing tools for its effective integration into the Czech healthcare system.
